Job summary

Alpha Home Health and Hospice is seeking a licensed Physical Therapist to provide direct rehabilitation care based on physician referrals. The role involves implementing and adjusting treatment plans, supervising PTA and student staff, documenting progress, and delivering high-quality patient service.

CPR must be kept current, and most responsibilities emphasize safety, confidentiality, and teamwork. Experience is preferred but new graduates are welcome to apply.

Qualifications

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT for 2003 or later).
• Current licensure in the applicable state, maintained actively.
• Preferably one year of experience; new graduates are welcome.
• CPR certification required upon hire and maintained during employment.

Responsibilities

• Implement and modify treatment plans, establish measurable goals, and evaluate patient progress.
• Supervise and assess care provided by PTAs, rehab aides, and students.
• Maintain accurate documentation, utilize therapy software, and ensure excellent patient service.
• Participate in community health initiatives and communicate effectively with patients, families, and team members.
• Follow safety, confidentiality, and company policies, demonstrating professionalism and integrity.
• Assist in patient evacuation and perform physical activities such as sitting, standing, lifting, and moving during work.
