Job description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patient’s physical abilities, analyzes rehabilitation goals, and consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships:

Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

1. Evaluate patients/residents within 24 hours of physician referral.
2. Develop effective treatment plans and obtain approval from referring physicians; treat patients/residents according to the approved plan.
3. Communicate regularly with supervisors, rehabilitation team members, and nursing staff regarding treatment plans, progress, and prognosis.
4. Record evaluations, treatment notes, progress reports, and discharge summaries as per policy.
5. Instruct patient/resident families or nursing staff in maintenance programs.
6. Secure necessary durable medical equipment to facilitate independence in mobility (e.g., orthoses, walkers, canes, elevated toilet seats, grab bars).
7. Submit logs and billing information promptly.
8. Report any issues with equipment or areas needing attention to the supervisor immediately.
9. Comply with the guidelines of the State Licensing Board of California, Title 22, CMS, and California Labor Board.
10. Perform other duties as assigned.

Qualifications:

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy.
