Job summary

Olde Maine Post Acute is seeking a licensed Physical Therapist to join our team. The role requires an active New Jersey license and entails evaluating residents, designing treatment plans, and delivering evidence-based rehab services to improve mobility and independence.

We offer a full-time, on-site position with competitive pay ($38-$45 per hour, all-inclusive) and a supportive interdisciplinary team focused on compassionate care, continuous learning, and quality outcomes.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current, active, NJ Physical Therapist license.
  • Evidence of satisfactory physical examination and freedom from communicable diseases.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to collaborate with interdisciplinary healthcare teams.
  • Commitment to evidence-based rehabilitation services.

Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

  • Monitor progress, modify treatment plans as needed, and document evaluations, treatments, and outcomes.
  • Communicate resident progress and recommendations with residents, families, physicians, nursing staff, and the interdisciplinary team.
  • Participate in care planning meetings and facility committees; promote professional development.
  • Maintain professional standards and protect resident rights and confidentiality.
  • Engage in continuing education to maintain clinical competency and stay current with best practices.

Responsibilities for Licensed Physical Therapist:
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders and resident goals.
  • Provide skilled physical therapy treatments designed to improve mobility, strength, balance, function, and overall quality of life.
  • Monitor resident progress, modify treatment plans as appropriate, and accurately document all evaluations, treatments, and outcomes.
  • Communicate resident progress, needs, and recommendations with residents, families, physicians, nursing staff, and other members of the interdisciplinary team.
  • Participate in care planning meetings, facility committees, and community education initiatives as appropriate.
  • Maintain professional standards of practice while protecting resident rights and confidentiality.
  • Participate in continuing education and professional development to maintain clinical competency and stay current with best practices.
Qualifications for Licensed Physical Therapist:
  • Graduate of an accredited Physical Therapy program required.
  • Current, active, and unencumbered Physical Therapist license in the state of New Jersey required.
  • Evidence of a current satisfactory physical examination and/or freedom from communicable diseases as required by state regulations.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to collaborate effectively with interdisciplinary healthcare teams.
