Job summary

Choice Health At Home is seeking a Physical Therapist to join our Peoria team. The role focuses on assessing and treating patients in home health, developing individualized programs, and ensuring documentation aligns with policies.

The ideal candidate holds a valid PT license, CPR certification, and at least 1 year of home health experience. A valid driver’s license and auto insurance are required.

Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, strengthen, and restore function.
• Perform and document an initial exam to identify problems and determine a diagnosis prior to intervention.
• Plan and carry out individually designed programs of physical treatment to maintain or restore physical function and alleviate pain.
• Evaluate treatment effects and adjust to maximize benefit.
• Apply infection control/universal precautions in patient care.
• Ensure continuity of quality patient care with proper documentation.

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

As a Physical Therapist, you will:

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Physical Demands
