Job summary

Qualifications

• 2 years experience in an acute care or rehabilitation setting.
• Graduate of an accredited physical therapy program.
• PT license in Oklahoma and BLS certification.
• English communication skills (verbal/written).
• Valid Oklahoma driver’s license and driving record acceptable to insurance carrier.

Responsibilities

• Interpret referrals; examine, evaluate, develop, and modify physical therapy plan of care with anticipated goals and outcomes.
• Perform comprehensive physical, psychosocial, and safety assessments of patients.
• Plan for and provide skilled physical therapy treatment in the home and reassess each visit.
• Coordinate patient care with other clinical team members and maintain clear documentation.
• Direct Physical Therapy Assistants and supervise home health staff.
• Maintain and submit all required patient documentation in a timely manner.

Responsibilities

• Interpret referrals; examine, evaluate, develop, and modify physical therapy plan of care including anticipated goals and expected outcomes.
• Perform comprehensive physical, psychosocial, and safety assessments of assigned patients as evidenced by clinical documentation.
• Plan for and provide skilled physical therapy treatment in the home, reassessing patients each visit and ensuring quality of care documented in the clinical record.
• Coordinate patient care with other clinical team members, ensuring optimal patient care and communication documented in the clinical record.
• Direct Physical Therapy Assistants to perform specific components of treatment and interventions, remaining responsible for the episode of patient care.
• Supervise and coordinate the delegation and implementation of patient care of licensed personnel, home health aides, and students.
• Maintain and submit all required patient documentation in a timely manner and in accordance with home health policies and procedures.
• Report to the Clinical Manager.
• Possibly required to meet additional physical demands, respiratory fit test, exposure to infections, blood and body fluids, electrical equipment, chemicals; must follow standard precautions.

Qualifications

Required Qualifications

• Experience: 2 years experience in an acute care or rehabilitation setting, with exposure to geriatric and medical/surgical clients desired.
• Education: Graduated from an accredited school of Physical Therapy program.
• License/Certifications: PT (Physical Therapist); BLS (Basic Life Support) issued by American Red Cross or American Heart Association.
• Must be able to communicate effectively in English (verbal/written).
• Must have a current Oklahoma State Driver’s License and a driving record acceptable to insurance carrier. Must operate a vehicle (INTEGRIS-owned or personal).
