Job summary

Valeo Home Health & Hospice seeks a compassionate Physical Therapist to provide skilled therapy in patients’ homes, helping them achieve rehabilitation goals and regain independence.

You will work with patients and caregivers to develop individualized care plans supporting function, safety, and quality of life, with supervision of PT Assistant every 30 days.

Qualifications

  • Graduate of an accredited physical therapy program (APTA or AMA approved).
  • Licensed as a Physical Therapist in the state of Indiana.
  • Minimum of 1 year of experience as a Physical Therapist.

Responsibilities

  • Provide skilled physical therapy in patients’ homes according to the physician’s plan of care, seeing multiple patients per day.
  • Evaluate and assess patient function and assist in developing and updating individualized care plans.
  • Deliver treatments to relieve pain, restore function, and support maximum performance using appropriate modalities.
  • Monitor patient progress, document outcomes, and communicate changes to the physician, DON, and care team.
  • Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive devices or equipment.
  • Reassess patient needs regularly and revise plans of care as appropriate.
  • Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
  • Participate in quality improvement initiatives, staff meetings, and ongoing education.
  • Provide required supervision of the Physical Therapy Assistant at least every 30 days.

Job Summary

We are seeking a compassionate Physical Therapist to join our team. Physical Requirements
  • Ability to sit, stand, and drive for extended periods while providing in-home care.
  • Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
  • Regular use of a tablet for documentation, communication, and training.
