BAYADA Home Health Care is seeking an experienced full Physical Therapist (PT) for home health visits throughout Orleans County, VT, and surrounding areas. One year of clinical experience as a licensed Physical Therapist is required. We offer both full-time and part-time opportunities.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Retention bonus for full-time positions; amount varies by territory.

Responsibilities include:

• Making home visits to clients in designated areas.
• Performing diagnostic tests and measurements, such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Developing and implementing individualized care plans, including manual exercises, gait training, and balance interventions.
• Assessing and revising care plans, collaborating with nursing staff as needed.
• Educating clients and family members on rehabilitative activities to promote health, safety, and independence.
• Documenting observations and interventions accurately using electronic tablets.

Qualifications:

• Current Vermont Physical Therapist (PT) license.
• Minimum of one year recent experience as a PT.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Computer proficiency; EMR experience preferred.
