Physical Therapist (PT) in Skilled Nursing Facility

Now staffing Physical Therapists for the Rehab Department in state-of-the-art SNFs throughout NYC and surrounding areas.

Flagstar Rehab PTs work with a diverse, multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve patients' physical and cognitive functions, enhancing their ability to perform daily activities.

Hourly Pay Rate: $42-$46 (W-2), $55-$57 (1099)

Physical Therapist Job Duties:

1. Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
2. Supervise PTAs daily.
3. Provide quality therapy services to individuals in an SNF setting, including cases related to TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery, and other debilitating diagnoses.
4. Monitor and document patient progress using innovative EMR software.

Physical Therapists employed by Flagstar Rehab will receive:
