Job summary

Aspire Senior Living New Florence is seeking a compassionate Physical Therapist (PT) to join on a per diem basis. You will assess and treat residents, develop individualized care plans, and collaborate with an interdisciplinary team to support mobility and quality of life.

In this role, you will provide group and individual therapy, educate residents and families, and maintain detailed progress notes to ensure continuity of care.

Responsibilities

• Assess and treat residents with a range of physical limitations and medical conditions
• Develop and implement individualized care plans, in collaboration with the care team
• Conduct thorough assessments and evaluations to identify areas for improvement
• Provide one-on-one and group therapy sessions, as well as equipment training and education
• Maintain accurate and detailed records of resident progress and treatment plans
• Collaborate with the care team to ensure continuity of care and communicate effectively with residents, families, and healthcare providers

About the Role:

We are seeking a compassionate and highly skilled Physical Therapist (PT) to join our team on a Per Diem basis. As a Physical Therapist (PT) at Aspire Senior Living New Florence, you will work closely with our interdisciplinary team to develop and implement individualized care plans that cater to the unique needs of our residents. Your Physical Therapist (PT) expertise will enable you to assess, treat, and educate residents on conditions such as arthritis, Parkinson's disease, and other mobility-related issues.
