Job description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patient’s physical abilities, analyzes rehabilitation goals, and consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships:

Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

1. Evaluate patients/residents within 24 hours of physician referral.
2. Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan.
3. Communicate regularly with supervisor, rehabilitation team, and nursing staff regarding treatment plans, progress, and prognosis.
4. Record evaluations, treatment notes, progress reports, and discharge summaries per policy.
5. Instruct patient/resident families or nursing staff in maintenance programs.
6. Secure necessary durable medical equipment to facilitate independence, such as orthoses, walkers, canes, and grab bars.
7. Submit logs and billing information timely.
8. Report equipment or problem issues to the supervisor immediately.
9. Comply with relevant licensing boards, CMS, and labor guidelines.

May perform other duties as assigned.

Qualifications:

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy.
