Job summary

The Physical Therapist position at Trinity Health in Muskegon, MI places you in the House Rehab Services department. You will evaluate patients and provide therapy within licensure, with day shifts Mon-Thu 8:00am - 4:30pm and Fri 9:00am-5:30pm; weekend coverage every four weeks and two holidays per calendar year.

The requirements include Doctorate in Physical Therapy (or equivalent), Bachelor's in Physical Therapy, FSBPT, MI license or eligible, and AHA BLS.

Qualifications

• Must be a Doctorate in Physical Therapy or equivalent (APTA-accredited).
• Bachelor's in Physical Therapy required.
• FSBPT Certification.
• State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
• American Heart Association BLS or equivalent.

Responsibilities

• Providing treatment and services within the scope of their license and as required by law.
• Evaluation and clinical diagnosis of patient conditions.
• Establish treatment goals with input from patient and family; documentation per standards.
• Participates in an interdisciplinary and patient-centered care team.

Day Shift, 8:00am - 4:30pm Monday - Thursday, 9:00am-5:30pm Friday, and 8:00am - 4:30 for Weekend Shifts

Weekend Coverage: Colleague will work one weekend (Saturday and Sunday) every four weeks

Holiday Coverage: Colleague will work two holidays per calendar year

This position is based in the House Rehab Services department in Muskegon, MI and does provide physical therapy services on an as-needed basis at Shelby Hospital in Shelby, MI.

Position Purpose:

The physical therapist will assume autonomous responsibility for evaluating patient's need for physical therapy.

What the Physical Therapist ( PT ) Will Do:

• Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.
• Evaluation and clinical diagnosis of patient conditions.
• Establish treatment goals utilizing input from patient and their family as appropriate Documentation remains in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.
• Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan

Minimum Qualifications:

• Must be a graduate of a Doctorate's or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.
• Required: Bachelor's in Physical Therapy
• FSBPT Certification.
• State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
• American Heart Association BLS or equivalent
