Job summary

Synchrony is seeking a Physical Therapist to provide a full range of PT services in an interdisciplinary setting. You will assess patients, plan and implement treatments, and educate patients and families on exercises and postural strategies.

The role emphasizes accurate documentation, collaboration with a care team, and adherence to professional standards. Licensure, CPR (preferred), and a PT degree from an accredited program are required.

Qualifications

• Must hold current state PT licensure or certification.
• CPR certification, current/valid, preferred.
• Degree in Physical Therapy from an accredited program.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans including exercises and manual therapy.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of progress and outcomes per facility policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Perform other duties as assigned.

Job Summary

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Education

• Degree in Physical Therapy from an accredited program

• 0-1 years

Roles and Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
