Job description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients' physical abilities and analyzes rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships:

Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

  1. Evaluate patients/residents within 24 hours of physician referral.
  2. Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan.
  3. Communicate regularly with supervisors, other rehabilitation team members, and nursing staff regarding treatment plans, progress, and prognosis.
  4. Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports, and discharge summaries per policy and procedure.
  5. Instruct patients'/residents' families or nursing staff in maintenance programs.
  6. Secure necessary durable medical equipment (e.g., orthoses, walkers, canes, elevated toilet seats, grab bars) to facilitate independence in mobility.
  7. Submit logs and billing information timely.
  8. Report any equipment or problem areas to the appropriate supervisor immediately.
  9. Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines.

Additional duties may be assigned as needed.

Qualifications:

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy.
