Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to join our field-based team in Texas. You will own the caseload in home health, delivering skilled PT visits and developing plans of care.

The role emphasizes gait training, transfers, strengthening, home safety, and caregiver coaching, with integration of our charting and scheduling tools to streamline care.

This full-time position offers flexible scheduling and a productivity-based compensation model.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment with field independence.
• Comfort escalating changes to RN/MD and documenting rationale clearly.
• Proficient in patient education and documentation.
• Must have valid driver’s license and daily travel across territory.
• Able to carry/transport basic therapy equipment.
• Competency in core home health PT scope: gait training, balance, transfers, home safety.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, mobility.
• Set measurable goals, track progress, and adjust treatment as needed.
• Coordinate with care team to keep plans aligned and on track.
• Supervise PTA where appropriate and document promptly in the EMR.
• Coach patients/caregivers with practical home programs.
• Document thoroughly in HCHB for defensible care.

Physical Therapist (PT) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): $75/PP

What you will do:

• Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

• Complete comprehensive in-home assessments and deliver skilled PT visits.

• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

• Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

• Manage treatment and outcomes directly, including PTA supervision where appropriate.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

Must-haves

• Active Texas PT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PT scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience
