Job summary

Interim HealthCare Inc. in Manchester, NH, is seeking a Home Health Physical Therapist to provide one-on-one care in patients' homes across the Portsmouth and Seacoast area. You will evaluate patients, develop individualized treatment plans, and work with an interdisciplinary team to restore strength, mobility, and independence.

The role offers flexible scheduling, autonomy over daily plans, ongoing CE opportunities, and a supportive leadership team committed to high-quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active New Hampshire Physical Therapist license.
• Minimum of one year of PT experience (home health experience preferred).
• Current CPR certification.
• Knowledge of state and federal home health regulations.
• Excellent clinical judgment, communication, and patient education skills.
• Compassionate, organized, and committed to delivering exceptional patient care.

Responsibilities

• Evaluating patients and developing individualized treatment plans.
• Restoring strength, mobility, balance, and functional independence.
• Providing hands-on therapy and therapeutic exercise.
• Monitoring progress and adjusting care plans as needed.
• Collaborating with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care.
• Educating patients and family members on exercises, safety strategies, and ongoing recovery.
• Completing accurate and timely clinical documentation.
