Job summary

Centra Rehabilitation Hospital, part of Lifepoint Health, is opening a new inpatient rehabilitation facility in 2026. We provide high-quality, compassionate care for adults and geriatric patients in a state-of-the-art, patient-centered environment.

Join our team as a Physical Therapist to lead patient care, collaborate with the interdisciplinary team, and contribute to a comprehensive patient plan of care while continuing education and growth opportunities are offered.

Qualifications

• Current licensure as a Physical Therapist in the state where the hospital is located.
• Inpatient rehab experience preferred.
• CPR/BLS certification required.

Responsibilities

• Assumes responsibility and accountability for the care of patients treated by Physical Therapists, Assistants, and therapy staff.
• Assess patients' needs and initiate appropriate physical therapy interventions; work with the patient care team to maintain professional standards.
• Observe and evaluate treatment progress; recommend changes to the physician when needed.
• Conduct patient assessments and reassessments; complete timely QA scoring as per policy; treat adult and geriatric rehab patients.

Centra Rehabilitation Hospital is a brand-new 50 bed inpatient rehabilitation facility opening in 2026. We offer high-quality, compassionate care for a wide variety of patients in need. Our teams provide evidence-based, transformational care for a broad range of injuries and illnesses in a compassionate, comfortable environment.

How You'll Contribute

A Physical Therapist who excels in this role:

• Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
• Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
• Observes and evaluates treatment affect, accomplishment of short and long term goals. Recommends change to physician, if needed.
• Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
• Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
• Formulates and updates patient specific plan of care for physical therapy
• Reports patient progress and barriers to discharge in each patient team conference.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate
• Performs appropriate documentation.
• Demonstrates knowledge, competency and proficiency of physical therapy modalities.
• Other duties as assigned.

What We're Looking For

Applicants should possess the following qualifications and skills:

• Current licensure as a Physical Therapist in the state where the hospital is located.
• Inpatient rehab experience preferred.
• Additional Requirements: CPR/BLS certification.

Hourly range: $35.00-$52.01 per hour.
