Choice Health at Home is seeking a Home Health Physical Therapist to provide skilled, one-on-one care in patients' homes across Boulder, CO. You will lead PTAs and collaborate with an interdisciplinary team to adapt treatment plans for optimal outcomes.

Successful candidates will hold a physical therapy degree and state license, with CPR certification and a valid driver’s license. HomeCare HomeBase experience is a plus, and a 1-year home health background is preferred.

Qualifications

• Completion of an accredited physical therapy program and state licensure exam.
• CPR certification is required.
• Valid driver’s license and auto insurance are required.

As a Home Health Physical Therapist, you’ll practice at the very top of your license, delivering skilled, one-on-one care directly in patients’ homes. No two days are the same, and every visit is an opportunity to create meaningful progress for someone in their most comfortable environment.

You’ll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible outcomes.

In this role, you will:

• Conduct comprehensive evaluations and establish clinical diagnoses to guide care

Working Hours

To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation.

Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours

are 8:00 am to 5:00 pm (CST).

Mileage

The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per

mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage

accurately and timely each pay period.
