Grammar LLC is seeking a Physical Therapist to join a team-based home health care environment. The role focuses on managing and delivering patient care plans with collaboration across care disciplines.

Responsibilities

  • Evaluate patients' physical condition, mobility, strength, balance, posture, and ROM.
  • Develop individualized treatment plans based on patient needs and goals.
  • Perform therapeutic exercises to improve strength, flexibility, endurance, and coordination.
  • Use manual therapy techniques such as joint mobilization and soft tissue manipulation.
  • Administer physical agents (heat, cold, ultrasound, electrical stimulation, traction) as appropriate.
  • Monitor progress and modify treatment plans based on outcomes.
  • Educate patients and caregivers on home exercise programs and injury prevention.
  • Collaborate with physicians, nurses, therapists to coordinate patient care.
  • Maintain accurate documentation of evaluations, sessions, progress, discharge notes.
  • Assist patients in regaining mobility after surgery, injury, stroke, or illness.
  • Recommend assistive devices and provide training on their use.
  • Assess fall risk and implement safety interventions.
  • Ensure compliance with healthcare regulations and privacy requirements.
  • Participate in discharge planning and coordinate follow-up care.
  • Supervise PTAs and rehab aides as permitted by state regulations.
  • Stay current with evidence-based practices through continuing education.
  • Schedule: Mon–Fri 8:00 am–6:00 pm, 1 weekend per month, and 2 holidays per year.

Physical Therapist Responsibilities
  • Evaluate patients' physical condition, mobility, strength, balance, posture, and range of motion.
  • Develop individualized treatment plans based on patient needs and goals.
  • Perform therapeutic exercises to improve strength, flexibility, endurance, and coordination.
  • Use manual therapy techniques, including joint mobilization and soft tissue manipulation.
  • Administer physical agents such as heat, cold, ultrasound, electrical stimulation, and traction when appropriate.
  • Monitor patient progress and modify treatment plans based on clinical outcomes.
  • Educate patients and caregivers on home exercise programs, injury prevention, posture, body mechanics, and self-management techniques.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care.
  • Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries.
  • Assist patients in regaining mobility following surgery, injury, stroke, or illness.
  • Recommend assistive devices such as walkers, canes, crutches, wheelchairs, or orthotics and provide training on their use.
  • Assess fall risk and implement interventions to improve safety and reduce injury.
  • Ensure compliance with healthcare regulations, ethical standards, infection control, and patient privacy requirements.
  • Participate in discharge planning and coordinate appropriate follow-up care.
  • Supervise Physical Therapist Assistants (PTAs) and rehabilitation aides as permitted by state regulations.
  • Stay current with evidence-based practices through continuing education and professional development.
  • Schedule: Monday – Friday 8:00 am – 6:00 pm, 1 weekend per month, and 2 holidays per year.
