Job summary

Qualifications

• Graduate of an accredited physical therapy program.
• Current state license in good standing.

Position Overview

The Physical Therapist (PT) provides direct rehabilitation care in physical therapy to patients in a skilled nursing facility setting.

Responsibilities

• Developing and implementing personalized treatment plans based on physician referrals
• Assessing and adjusting goals to promote patient progress and functional improvements
• Supervising and evaluating care provided by PT aides, PTAs, and students
• Documenting patient progress accurately and timely using therapy software
• Collaborating with interdisciplinary team members to ensure comprehensive patient care
• Maintaining professional relationships with patients, families, and staff
• Adhering to safety, confidentiality, and compliance standards
• Participating in community health initiatives as appropriate

Qualifications & Skills

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); new grads are welcome
• Current state license in good standing
• CPR certification (obtained upon hire or during orientation)
• Proficiency in Microsoft Office and email communication
• Strong interpersonal, decision-making, and body mechanics skills
