Job summary

Interim HealthCare of Allentown is seeking a Home Health Physical Therapist (PT) for a full-time role serving patients in Allentown. You will deliver personalized therapy in patients’ homes, focusing on functional outcomes and safe mobility, with a strong emphasis on measurable progress.

You will evaluate and plan care, perform therapeutic exercises, supervise assistants, and educate families. Licensing in PA, CPR, and a valid driver’s license are required; two years of PT experience is

Qualifications

• Graduate from a CAPTE-accredited physical therapy program and hold a PA PT license.
• Two years of PT experience in relevant patient populations preferred.
• Home health experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate function, mobility, strength, balance, safety and equipment needs in the home.
• Develop plan of care with measurable goals and timelines.
• Provide therapeutic exercise, gait and transfer training per plan of care.
• Reassess at each visit and obtain revised orders as needed.
• Educate patients and caregivers on exercises, fall prevention and safe mobility.
• Supervise PT assistants as assigned.
• Document treatment and progress accurately and support discharge planning.

What You'll Do

As a Home Health PT, you will deliver personalized therapy that helps patients move safely and confidently at home:

• Evaluate function, mobility, strength, balance, safety, and equipment needs in the home
• Help develop the plan of care with measurable short- and long-term goals
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care
• Reassess at each visit, complete re-evaluations, and obtain revised orders when indicated
• Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility
• Supervise physical therapist assistants as assigned
• Document treatment, response, and progress accurately and on time, and support discharge planning

What You Bring

• Graduate of a CAPTE-accredited physical therapy program with a current Pennsylvania PT license
• Two years of physical therapy experience relevant to the patient population within the past five years preferred
• Home health experience preferred
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation
• Strong clinical judgment, communication, organization, and documentation skills
