Job summary

Trilogy Health Services is seeking a Physical Therapist to provide comprehensive PT services at our US-IN Jasper location. You will assess, plan, and administer interventions to improve patients’ function in collaboration with an interdisciplinary team.

The role requires a Master’s degree in Physical Therapy with current state licensure; CPR is preferred. Travel may be required, and benefits include competitive pay and health coverage.

• Must hold a current, valid state PT license.
• CPR certification is preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of progress and outcomes.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Perform other duties as assigned.

POSITION OVERVIEW

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
