Job summary

Trilogy Health Services—Amber Manor Care Center in Petersburg, IN—is seeking a Physical Therapist to provide a full range of PT services within an interdisciplinary team. You will assess, plan, and implement individualized interventions to improve patient function.

Responsibilities include therapeutic exercises, manual therapy, education for patients and families, and meticulous documentation. Requires Master’s in Physical Therapy and active state licensure; CPR preferred. Travel may be required.

Qualifications

• Master's degree in Physical Therapy required.
• Active state PT licensure in Indiana required.
• CPR certification preferred.

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with therapeutic exercises and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate progress documentation in accordance with policies.
• Collaborate with interdisciplinary care team to ensure comprehensive patient care.
• Perform other duties as assigned.

Position Overview

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
