Job summary

Avante at Inverness Skilled Nursing and Rehabilitation Center is seeking a full-time Physical Therapist to oversee resident assessments and coordinate therapy services in our in-house department. You will ensure compliance with federal, state, and local regulations and work under physicians' orders to deliver high-quality care.

The ideal candidate holds a BS in Physical Therapy, has passed the National Certification Examination for Registered Physical Therapist, and holds or is eligible for

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• 0-3 years of long‑term care experience.

Responsibilities

• Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.
• Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implements and conducts treatment as outlined in treatment plan.
• Follows management direction.
• Performs other duties as assigned.

Job Function

Provides and directs Physical Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under physicians' orders.
