Job summary

InHome Therapy is hiring licensed home care Physical Therapists (PT) to serve adult and geriatric patients in their homes in and around Hoffman Estates. You will work with a flexible schedule, a consistent caseload, and a supportive team that values patient-centered care.

The role involves evaluating mobility, developing individualized treatment plans, implementing therapeutic exercises, educating patients and families, and maintaining Medicare-compliant documentation.

Qualifications

• Active PT license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in homes.
• Comfort with email, texting and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients.
• Evaluate mobility, strength, and function to develop treatment goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain Medicare and agency documentation per standards.

The evaluation visit rate for this role ranges from $65 - $85 per visit

Key Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community
• Evaluate patient mobility, strength, and function to develop individualized treatment goals
• Implement effective rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare and agency standards

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Basic comfort with technology - email, texting, EMR systems (training provided)

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
