VitalCaring Group in Texas is seeking a Home Health Physical Therapist to help patients regain mobility and independence in the comfort of their homes. You’ll conduct thorough assessments, create individualized care plans, and collaborate with physicians and the care team to deliver patient-centered rehab.

This highly autonomous, relationship-driven role offers flexible scheduling, ongoing clinical education, and benefits including 401(k) with company match, FSA/HSA options, and paid time off.

Qualifikationen

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
