Rock Health is seeking a licensed Physical Therapist (PT) to develop and implement individualized treatment plans aimed at enhancing patient recovery within our clinical team. You will assess functional needs, adjust care, document progress, and collaborate with other healthcare professionals to achieve optimal outcomes.
The role requires graduation from an APTA-approved program, current state licensure, and at least one year of experience in a physical therapy aide role.
Role & Responsibilities: The Physical Therapist develops and implements individualized treatment plans to enhance patient recovery, assessing functional needs and adjusting care accordingly. They provide direct patient care, collaborate with healthcare professionals, and communicate effectively to achieve positive outcomes. Responsibilities include documentation, maintaining standards of care, and guiding support staff. They also promote professional growth through ongoing education and serve as a resource within the community.
Qualifications & Skills: Candidates must have a degree from an APTA-approved program, a current state license, and at least one year of experience as a physical therapy aide. Essential skills include the ability to care for diverse age groups, remain calm in crises, interpret instructions accurately, and work independently with integrity. Knowledge of safety protocols, infection control, and resident confidentiality (HIPAA) is required.
Key Duties: Evaluate patient needs, design and adjust care plans, document treatments, and coordinate with team members. Ensure compliance with standards of care, safety, and regulatory policies, while fostering professional development and community engagement.
