Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist (PT).
• CPR certification (preferred or required depending on the facility)

Responsibilities

• Perform thorough assessments of patients' physical abilities and pain levels; develop and implement individualized treatment plans.
• Deliver evidence-based PT interventions to improve strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, and other therapists to coordinate care.
• Monitor progress, adjust plans as needed, and document care per guidelines.
• Educate patients and caregivers on safe mobility techniques and home exercises.
• Ensure compliance with Medicare/Medicaid documentation and facility policies.
• Participate in care planning meetings and interdisciplinary conferences.

Job description

The Physical Therapist is responsible for evaluating residents' physical conditions, developing personalized treatment plans, and delivering rehabilitative care to help patients improve mobility, restore function, and achieve the highest possible level of independence.

Key Responsibilities

• Perform thorough assessments of patients' physical abilities, movement limitations, and pain levels. Develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals.
• Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, occupational and speech therapists, and family members to coordinate patient care.
• Monitor patient progress, adjust therapy plans as needed, and document all care provided in accordance with state and federal guidelines.
• Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use.
• Ensure compliance with all regulatory requirements including Medicare/Medicaid documentation and facility policies.
• Participate in care planning meetings and interdisciplinary team conferences.
• Supervise physical therapist assistants (PTAs) and aides as needed.
• Maintain a safe and clean work environment.
