Job summary

SunnyBrook Home Care in Fairfield, IA seeks a licensed Physical Therapist to join our home health team. The role involves directing the therapy team to ensure optimal patient outcomes and delivering high-quality home visits 32–40 hours per week.

The PT will perform assessments, develop care plans under physician direction, and promote interdisciplinary collaboration. Relocation assistance is available for the right candidate and part-time options may be considered with PTA support.

Qualifications

• Graduate of a physical therapy degree from a CAPTE-accredited program.
• Currently licensed as a Physical Therapist in the state of Iowa.
• At least one year of full-time work experience in a hospital, long-term care, or home care setting.

Responsibilities

• Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
• Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.
• Evaluates and assesses the patient’s level of functioning by applying appropriate tests.
• Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the plan of care.
• Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient’s care delivery.
• Ensures quality and safe delivery of services to patients according to the care plan.
• Promotes patient independence by teaching patients and family members to understand treatment goals and plan.

SunnyBrook Home Care is looking for a Physical Therapist to join our team. This position is 32-40 hours per week. Relocation assistance possible for the right candidate. We are also willing to hire part-time with the support of the PTA. We pride ourselves on teamwork and flexibility!

This is an opportunity to direct the therapy team to ensure the best outcome for each patient.

The Physical Therapist uses clinical expertise, physical assessment, teaching and skilled therapy interventions to deliver high quality patient care in the patient’s home. The PT develops or contributes to the development of the home health plan of care under the direction of the physician and facilitates appropriate interdisciplinary involvement.

Experience

• Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen
• Valid driver’s license, reliable transportation, valid insurance, and clean driving record
• At least one year of full-time work experience in a hospital or long-term care setting and/or home care experience required
• Graduate of a physical therapy degree from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education
• Currently licensed as a Physical Therapist in the state of Iowa.
• Candidates must excel in an autonomous work environment, be highly motivated, detail orientated, organized and technologically proficient.
