Elizabeth Park Rehab is seeking a PRN Physical Therapist to join our skilled nursing facility team in New Jersey. The PT will assess and treat residents, develop care plans, and participate in data collection and documentation using Net Health/PCC EMR.

Responsibilities

• Individualized Care Plans: Develop plans with goals for mobility and pain reduction.
• Section GG Data Collection: Accurately collect data for Annual, Quarterly, Falls, and Significant Change assessments.
• Treatment: Administer PT interventions to improve function.
• Education: Educate residents, families, and staff on safe mobility techniques and progress.
• Documentation: Maintain up-to-date documentation in EMRs (Net Health/PCC).
• Interdisciplinary Collaboration: Work with rehab team including OTs and SLPs.

Physical Therapist, PT Job Description

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Physical Therapist, PT Responsibilities

• Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
• Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
• Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
• Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
