Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time role performing home health visits in Gloucester County, NJ, based from our West Deptford office. New grads are welcome to apply, and you will join a multidisciplinary team to deliver rehabilitative care in clients' homes.

Responsibilities include comprehensive assessments, personalized care plans, and education for clients and families. Strong documentation and EMR use are required.

Qualifications

• A current New Jersey Physical Therapist license.
• Graduation from a program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Develop and implement individualized care plans with manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions using a tablet-based system.

BAYADA at Inspira Home Health is currently seeking an experienced Physical Therapist, for full-time opportunity performing home health visits for our Gloucester County office, located in West Deptford, New Jersey. This office services clients on a per visit basis in territories throughout Gloucester County, NJ.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current New JerseyPhysical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$60.00-$70.00 per point ($93,602-$109,202 annual earning potential)
