Job summary

Blue Palms Health & Rehabilitation Center is seeking a licensed Physical Therapist to join our subacute rehabilitation team in Deland, FL. You will design and implement customized treatment plans to help patients regain mobility and independence, while educating families on recovery strategies.

Collaborate with nurses, therapists, and physicians to deliver patient-centered care, track progress, and ensure regulatory compliance.

Qualifications

• Active Florida license as a Physical Therapist.
• Experience in subacute or skilled nursing settings preferred.
• Strong communication and clinical skills.
• Ability to work independently while collaborating with a team.

Responsibilities

• Create customized treatment plans to restore mobility and strength.
• Assess, treat, and educate patients and families on recovery.
• Collaborate with nurses, therapists, and physicians for patient-centered care.
• Track progress and maintain regulatory compliance.
• Occasional weekend coverage may be required.
