Job summary

Ridgecrest Health & Rehab in DeLand, FL is seeking a dedicated Physical Therapist (PT) to join our compassionate rehab team on a PRN/Full Time basis. You will assess patients, develop individual treatment plans, and collaborate with physicians, nurses, and therapists to maximize functional outcomes.

Ideal candidates hold an active Florida PT license, CPR certification, and have at least one year of experience in a Skilled Nursing Facility.

Qualifications

• Active Florida license as a Physical Therapist is required.
• CPR certification is required.

Responsibilities

• Creates and implements individualized PT treatment plans.
• Communicates with patients, families and healthcare team.
• Maintains documentation per facility requirements and payer needs.
• Develops and enhances clinical and professional skills.
• Supports department operations and development.
• Reports changes in patient status to Rehab Director.

Summary of the Physical Therapist (PT)

The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Job Requirements of Physical Therapist (PT)

• Current Florida License as a PT (Physical Therapist)
• Current CPR
• At least one (1) experience working in a Skilled Nursing Facility preferred.

Essential Duties & Responsibilities of the Physical Therapist (PT)

• Creates, implements, and executes patient treatment plans.
• Communicates with patients, families, referring physicians, and other members of the team.
• Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
• Develops and enhances clinical and professional skills through knowledge and professional associations.
• Supports and participates in department operations and development.
• Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
• Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
