Job summary

InHome Therapy is seeking a licensed Physical Therapist to provide in-home care for adults and geriatric patients in Cape Coral and surrounding areas. You will assess mobility and functional goals, design individualized plans, and implement therapeutic exercises in patients\' homes.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.
• Proficiency with email, texting, and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy to adults and geriatric patients.
• Evaluate mobility, strength, and function to develop goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain Medicare-compliant home health care documentation.

Compensation and Benefits*

• The evaluation visit rate for this role ranges from $65-80 per visit

Key Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community
• Evaluate patient mobility, strength, and function to develop individualized treatment goals
• Implement effective rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare and agency standards

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Basic comfort with technology - email, texting, EMR systems (training provided)

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
