Salary based on full-time employment and max productivity: $100,000 to $115,000 per year.

Responsibilities

The Home Health Physical Therapist (PT, DPT, RPT) is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and needs. Assist in developing the therapy plan of care with the physician.
• Observe and report patient responses and condition changes to the supervising nurse or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Instruct and inform the healthcare team, patient, and family/caregivers about the plan of care, functional limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to dependable transportation.
