Silver Creek Manor in Bristol, Rhode Island is seeking a Physical Therapist PT for full-time, part-time, or PRN opportunities. The role offers $45-$60/hour depending on experience, with sign-on and referral bonuses and flexible shifts.

Qualifications

• Valid RI license/registration required.
• Experience in long-term care preferred.

Responsibilities

• Assessment of referred patients requiring rehabilitation.
• Direct patient care to assess medical condition, function, limits and rehab potential.
• Establish and administer treatment program with goals per patient capacity.
• Set realistic goals for patients.
• Document and educate patients and families in PT methods.
• Perform evaluations, weekly progress notes, recertifications and discharge docs.
• Collaborate with all disciplines for patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA and Residents' Rights.

We are seeking a dedicated Physical Therapist PT to help us deliver outstanding rehabilitation services to our residents.

Your Role as a Physical Therapist PT

At Silver Creek Manor, our Physical Therapist PT provides essential care to our residents by:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

What You Need

• Graduated from an accredited Physical Therapy college or university

• Valid license and registration for the state of RI

• Experience with long-term care preferred
