Summary of the Physical Therapist (PT):

The Physical Therapist will develop individualized treatment programs, including assessment, treatment, planning, education, and communication, to help patients achieve their functional goals. The PT will collaborate with other health professionals and staff to ensure optimal patient care.

Qualifications

Job Requirements:

• Current Florida License as a PT (Physical Therapist)
• Current CPR certification
• At least one year of experience working in a Skilled Nursing Facility (preferred)

Responsibilities

Essential Duties & Responsibilities:

• Develop and implement patient treatment plans.
• Communicate effectively with patients, families, physicians, and team members.
• Maintain timely documentation as per facility, department, and insurance requirements.
• Enhance clinical and professional skills through ongoing education and professional associations.
• Support department operations and development.
• Report changes in patient status to the Director of Rehab and interdisciplinary team.
• Supervise Physical Therapy Assistants to ensure quality patient care and outcomes.
