Job summary

Post Rehab is looking for a Physical Therapist (PT) to join their rehabilitation team at the Agawam Campus in Massachusetts. This full-time position allows for collaboration with interdisciplinary team members and focuses on enhancing the mobility and independence of residents in both short-term rehab and long-term care.

The ideal candidate will possess an active Physical Therapist license, and strong communication and clinical reasoning skills. The position offers competitive hourly pay and a range of benefits including medical, dental, vision insurance, and paid time off.

Qualifications

Responsibilities

• Provide therapy focused on gait training, transfers, balance, strengthening, endurance, and pain management.
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident goals.
• Monitor resident progress and adjust treatment plans as clinically appropriate.
• Maintain timely and accurate documentation in accordance with facility standards.

Agawam Campus – Agawam, MA – $45–$50/hour

Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This role offers the opportunity to work closely with residents in both short-term rehab and long-term care while helping improve mobility, strength, balance, and functional independence.

Position Responsibilities

• Complete physical therapy evaluations and establish individualized treatment plans
