Job summary

Choice Health at Home is seeking a PRN Physical Therapist in Altamonte Springs, FL to assess, plan, and participate in rehabilitative programs for home health patients.

The role requires completion of an accredited PT program and a valid PT license, CPR certification, and a valid driver’s license with auto insurance. HomeCare HomeBase experience is a plus.

Responsibilities

• Maintain infection control and proper documentation.
• Communicate effectively with patients, families, and health-care team.
• Adhere to agency policies and maintain documentation per guidelines.

Position Summary

Assess , plan, organize , and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury .

As a Physical Therapist , you will:

Perform and document an initial exam , evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license

1 year of home health experience preferred

CPR certification

Valid Driver’s license

Valid Auto Insurance

HomeCare HomeBase experience preferred

Physical Demands

Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

Must be able to lift, carry, pull, or push up to 100 lbs..
