General Purpose
To ensure the highest quality of care while providing therapy services to patients and supervising therapy assistants, in accordance with all relevant standards and regulations.

Essential Job Functions

1. Provides physical therapy services based on a physician’s plan of care, including assessment, treatment, patient education, and reporting.
2. Instructs patients, families, and caregivers on therapy use and goals, participating in case conferences.
3. May train in prosthetic device use.
4. Prepares clinical and progress summaries, participates in discharge planning, and maintains communication with care team.
5. Attends in-services, case conferences, and meetings for coordinated patient care.
6. Identifies patient and caregiver needs and makes appropriate referrals.
7. Supervises physical therapy assistants per policies and regulations.
8. Adheres to all company policies and performs other duties as assigned.

Qualifications

• Degree from an accredited physical therapy program (bachelor’s or master’s).
• Valid license to practice in the state.
• Minimum of 2 years of clinical experience.
• Good communication and organizational skills.
• Familiarity with computer use, including internet, email, and confidentiality practices; experience with Apple devices is a plus.
• Valid driver’s license with insured vehicle; some travel may be required.
• Ability to perform job functions with or without accommodations.
