Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist to join a clinician-led team in Little Chute, Wisconsin. You’ll evaluate and treat patients, with scheduling that fits daytime hours and a focus on collaboration with other disciplines.

The role requires a DPT, current WI license, NPTE, and about 1 year of experience preferred; you’ll educate patients on home programs and injury prevention while maintaining high standards of care.

Qualifications

• Doctor of Physical Therapy (DPT) degree is required.
• Current, unencumbered state Physical Therapy License (WI); NPTE must be passed.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:
