Job summary

Charlestown by Erickson Senior Living is hiring a resident-centered Physical Therapist with weekday availability. All residents live on campus, allowing you to participate in their journey across the continuum of care.

The role supports residents in all aspects of daily living with person-centered care, requiring SNF experience and a state license. Inpatient/outpatient experience is preferred, with a strong emphasis on geriatric care and accurate documentation.

Qualifications

• Inpatient and Outpatient experience preferred to support residents across care settings.
• Therapeutic care for older adults across multiple diagnostic groups preferred.
• Must meet Medicare and state requirements for degree and training.

Responsibilities

• Provide holistic, person-centered physical therapy to residents.
• Implement residents’ care plans focusing on strengths, preferences, and routines.
• Collaborate with an interdisciplinary clinical team to address resident needs throughout their care.

Location: Charlestown by Erickson Senior Living

We are hiring a resident-centered Physical Therapist with weekday availability to join our team. All of our residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care.

Our Physical Therapists support our residents in all aspects of daily living, providing person-centered care to ensure we meet all of our resident’s needs.

Skilled Nursing Experience Required

Compensation

$55.00 per hour.

• Providing a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs.
• Implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.
• Working with the interdisciplinary clinical team to ensure the needs of our residents are being addressed throughout their care.

What we require:

• Degree and training must meet current Medicare and state requirements.
• Must possess a valid and current state Physical Therapy license.
• Inpatient and Outpatient experience highly preferred.
• Experience and the ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
