Job summary

LMH Health is seeking a Physical Therapist to provide comprehensive evaluations, modalities, and treatments for a designated patient group in alignment with hospital policy and high-quality care standards.

The ideal candidate will perform patient assessments, establish care plans, and educate patients and families while collaborating with the care team to ensure optimal outcomes.

Qualifications

• Required: Entry Level Bachelor’s Degree, Master’s Degree or Doctorate in Physical Therapy from accredited program.
• Current license through Kansas State Board of Healing Arts BLS Certificate

Responsibilities

• Performs patient assessment and establishes a plan of care.
• Assesses effectiveness of treatment and modifies plan to achieve goals.
• Provides care in accordance with physician’s orders and plan of care.
• Assesses educational needs of patient, family, caregiver, or significant others and provides education.
• Communicates honestly with others and resolves problems as they arise.
• Assumes additional responsibilities to support team efforts.
• Participates in continuous performance improvement processes.
• Embraces changes to improve patient-centered care.
• Performs care according to population served as per policy.
• Performs other duties as needed or assigned.

Education

Bachelor’s / Master’s / Doctorate in Physical Therapy (accredited program)

Current license through Kansas State Board of Healing Arts

Job Summary

The Physical Therapist assumes responsibility and accountability for a designated group of patients and provides physical therapy evaluations, modalities, and treatments in accordance with established hospital and departmental policy and produces, maintaining the highest degree of quality patient care.

Essential Job Responsibilities

• Provides care in accordance with the physician’s orders and established plan of care.
• Assess educational needs of the patient, family, caregiver, or significant others and provides education to meet those needs.
• Communicates honestly with others and discusses issues and resolves problems as they arise.
• Willingly assumes additional responsibilities to support team efforts.
• Actively participates in continuous performance improvement processes.
• Willingly embraces changes that will improve customer focused care.
• Performs care according to the population served on the unit as referenced in the Administrative, Scope of Services Policy.
• Performs other duties as needed or assigned.
