Job summary

Brushy Creek Post Acute in Greer, SC, seeks a compassionate Physical Therapist (PT) for a PRN position to evaluate and treat residents, develop therapy plans, and collaborate with physicians, families, and the rehabilitation team to maximize mobility and independence. The ideal candidate holds a Bachelor's in Physical Therapy with SC licensure, plus experience in skilled nursing or rehab settings.

Proficiency with Net Health and PCC is preferred.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Current Physical Therapist license in South Carolina and in good standing.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and established goals
• Assess functional abilities, mobility, strength, balance, and endurance
• Monitor resident progress and modify treatment plans as appropriate
• Communicate effectively with residents, families, physicians, nursing staff, and interdisciplinary team members
• Complete accurate and timely documentation in compliance with facility, state, federal, and payer requirements
• Participate in care planning meetings and discharge planning activities
• Educate residents, families, and caregivers on treatment plans, safety techniques, and home exercise programs
• Conduct in-service education and training for facility staff as needed
• Recommend adaptive equipment and interventions to improve safety and independence
• Maintain compliance with all facility policies, state practice acts, and regulatory requirements
• Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies

Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Education & Experience

• Bachelor's degree in Physical Therapy required
• Master's or Doctorate degree in Physical Therapy preferred
• Experience in skilled nursing, rehabilitation, or long-term care preferred
• Proficiency with Net Health and PointClickCare (PCC) preferred
• Strong clinical, communication, and documentation skills

Licenses & Certifications

• Current Physical Therapist license in South Carolina and in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the applicable State Board
