Job summary

Shore Gardens Rehabilitation and Nursing Center is seeking a caring Physical Therapist for a PRN/part-time role in Ocean County, New Jersey. The PT will evaluate and treat residents, communicate with families and the health team, and maintain all documentation in medical records.

Responsibilities include evaluating conditions, developing rehab plans, improving mobility, and reducing pain using therapies and manual techniques. SNF experience is preferred but not required; new grads welcome.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New Jersey PT License.
• 1 year experience preferred; new grads considered.
• SNF experience preferred, not required.
• Hands-on Rehab Optima (Net Health) experience preferred.
• Strong communication and professional behavior.

Responsibilities

• Evaluate residents' conditions and develop rehabilitation plans.
• Improve mobility and relieve pain through modalities, exercises, and stretching.
• Instruct residents on the use of physical therapy equipment.
• Coordinate with rehab team and health professionals to optimize treatment.
• Submit billing logs and information timely; supervise up to 2 PTAs and cosign notes.
• Maintain resident confidentiality and comply with legal requirements.

The In-House Therapy Team at Shore Gardens Rehabilitation and Nursing Center, a premier long term skilled nursing facility located in Ocean County, New Jerseyis actively looking for a caring, dedicated Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

PT Duties Include:

• Evaluate Residents conditions & develop appropriate physical rehabilitation plans.
• Help Residents improve their mobility.
• Relieve Resident’s pain using modalities, hands-on therapy, therapeutic exercises & stretching.
• Teach Residents how to use physical therapy equipment.
• Help Residents through their tasks and monitor their performance.
• Maintain Resident confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Submit all billing logs and necessary information on a timely basis.
• Supervising up to 2 PTA's and cosigning all notes

PT Requirements Include:

• Graduate of an accredited Physical Therapy Program
• Current New Jersey PT License
• 1 Year working experience as a Physical Therapist preferred, but new grads considered.
• SNF Experience preferred, but not required
• Hands-on experience with Net Health (Rehab Optima) preferred
• Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession
• Professional behavior
• Ability to manage patients with different types of personalities
• Excellent interpersonal communication skills
