Job summary

About this position

Physical Therapist PRN will provide direct patient care to home bound patients. Pay will be per visit.

Responsibilities

Develops and revises plan of care in consultation with physician and other care‑team members.
Provides therapeutic treatments and evaluation of equipment needs as applicable.
Participates in clinical conferences and in‑service programs.
Conducts on‑site supervisory visits of physical therapy assistants.
Advises and consults with the family and other agency personnel as applicable.
Prepares clinical and progress notes.
Protects and honors patient and coworker confidentiality and right to privacy.
Evaluates outcomes of care.
Plans for discharge.
