Job summary

PARADIGM REHAB & HEALTH CARE LLC is looking for a Physical Therapist PRN to provide direct care to homebound patients. The role involves developing care plans, conducting treatment evaluations, and supervising therapy assistants.

The ideal candidate will have a current Texas PT License, current CPR Certification, and a valid Driver's License. Compensation will be per visit and includes mileage reimbursement for travel.

Qualifications

Licensed Physical Therapist with a current Texas PT License.
Valid Texas Driver's License and Auto Insurance is required.
Current CPR Certification is necessary.

Responsibilities

Develops and revises plan of care in consultation with physician.
Provides therapeutic treatments and equipment evaluations.
Conducts on-site supervisory visits of therapy assistants.

About this position

Responsibilities and Duties

Develops and revises plan of care in consultation with physician and other care-team members.
Provides therapeutic treatments and evaluation of equipment needs as applicable.
Participates in clinical conferences and in-service programs.
Conducts on-site supervisory visits of physical therapy assistants.
Advices and consults with the family and other agency personnel as applicable.
Prepares clinical and progress notes.
Protects and honors patient and coworker confidentiality and right to privacy.
Evaluates outcomes of care.Plans for discharge.
