Parx Home Health Care is currently seeking a dedicated and compassionate Physical Therapist (PT) to join our team on a PRN (as needed) basis. In this role, you will have the opportunity to provide essential physical therapy services directly in patients' homes, ensuring they receive the highest quality of care tailored to their individual needs. You will collaborate with our multidisciplinary team to design and implement rehabilitation programs that enhance mobility, improve quality of life, and facilitate recovery. Responsibilities

• Perform thorough evaluations of patients' physical and functional abilities, developing individualized treatment plans
• Deliver physical therapy interventions and modify treatments based on patients' progress and feedback
• Educate patients and caregivers on exercises and techniques that promote healing and prevent future injuries
• Document patient progress and maintain accurate records in compliance with insurance and regulatory standards
• Collaborate with a team of healthcare professionals, including nurses and physicians, to coordinate care efficiently
• Participate in the continuous improvement of clinical practices to ensure effective patient outcomes
  
  
  

Requirements

• Possession of a valid Physical Therapist license in the state of Florida
• Graduation from an accredited physical therapy program
• Experience in a clinical setting; prior home health experience is a plus
• Strong interpersonal skills to effectively communicate with patients and their families
• Ability to work independently, manage time effectively, and adapt to changing patient needs
• Understanding of physical therapy principles, methodologies, and best practices
