Job summary

Encore Village in Illinois is seeking a PRN Physical Therapist to join our outpatient department. The Physical Therapist assesses, develops, and delivers therapy programs to facilitate rehabilitation, and identifies referrals to other disciplines to address comprehensive patient needs.

They implement individualized programs to restore and promote independence, educate patients and caregivers, and direct activities to promote health and mobility.

Qualifications

• Graduate of an accredited PT program.
• Current IL State Licensure.
• CPR Certified.
• Proficient with NetHealth therapy documentation.

Encore Village is currently hiring for a PRN Physical Therapist. Looking for our outpatient departments. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. They implement individualized therapy programs designed to restore, reinforce and enhance performance. In additiona, the Physical Therapist instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, to diminish or correct pathology. They direct activities to promote and maintain health.

Qualifications:

• Graduate of an accredited Physical Therapist (PT) therapy program
• Current IL State Licensure
• CPR Certified
• Proficient with therapy documentation (NetHealth)
