Position Summary

Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.

MAJOR CHALLENGES:

  • Responding to changes in health care delivery system.
  • Promoting team cohesiveness interdepartmentally and intradepartmentally.
  • Maintaining expertise in a field with a varying diagnostic populations, changing health care environment and rapidly advancing technology through constant study and review.
  • Providing a variety of complex services using good organizational and time management techniques.
  • Providing patient care in the face of patient mental compromise or foreign language barriers.

Patient Population Served: Adolescent, Adult, Geriatric. Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.

Position Specific Responsibilities

  • Assesses patients.
  • Plans and administers medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part.
  • Completes comprehensive physical assessments and identifies need for referral to additional appropriate clinical disciplines.
  • Performs patient tests, measurements, and evaluations such as range-of-motion and manual muscle tests, gain and functional analysis, and body-part measurements.
  • Records and evaluates tests, measurements, and evaluations to aid in establishing or revising specifics of treatment programs.
  • Administers manual therapeutic exercises to improve or maintain muscle function.
  • Instructs, motivates, and assists patients in non-manual exercises, such as isometric and progressive-resistive exercises; and in functional activities, using weights, pulleys, exercise machines, amts, steps, and assistive and supportive devices such as crutches, canes, parallel bars, and prostheses.
  • Administers treatments involving applications of physical agents such as heat/cold, massage and other modalities under a physician’s order.
  • Confers with physician and other health practitioners to obtain additional patient information, suggest revision in treatment programs, and integrate physical therapy with other aspects of patient health care.
  • Instructs patient and family in physical therapy procedures to be continued at home.
  • Supervises licensed physical therapy assistants and aides.

Education and/or Experience

  • Required - Bachelor's Degree From an accredited Physical Therapy Program Or
  • Required - Master's Degree From an accredited Physical Therapy Program.
  • Required - D.P.T. From an accredited Physical Therapy Program.
  • Required - 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Computer Skills: Laptop skills; basic knowledge of Windows operating system.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy. Reliable transportation required. Adapt to frequent change of work environment and patient/client population. Interact positively with large multidisciplinary team. Establish therapeutic relationships with patient and caregiver. Process information accurately, thoroughly and quickly. Prioritize tasks and manage time to meet department productivity and standards. Read and write therapy reports, clinical notes, and communications to and from other professional team members. Concentrate and attend to detail amidst a variety of environmental distractions. Person often works independently and autonomously. Ability to travel efficiently. Ability to deal with a variety of patient/client ages, behavior and cognitive levels.

Certifications, Licenses, and Registrations

  • Required upon hire: Washington Physical Therapist License (Vendor Managed)
  • Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)
  • Required within 30 days of hire: National Provider Identifier (NPI) number

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. This description provides a general overview and may vary by role and department, capturing the nuanced demands and conditions inherent to clinical positions in our organization.

Compensation range: $47.26-$63.26

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
