Qualifications

• Education: BS or higher in Physical Therapy.
• Current state license/certification.
• Active registration with ABPT (American Physical Therapy Certification Board).
• Prefer minimum one year of rehabilitation experience.
• Excellent oral, written, and interpersonal communication skills.
• Willingness to provide therapy in SNF, outpatient, long-term care, or home health settings.
• Demonstrate competency with PT methods, materials, and equipment.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Evaluate and treat patients per physician orders using therapeutic procedures.
• Collaborate with care teams to restore mobility and function.
• Adapt therapy plans for Skilled Nursing, Outpatient, Long Term Care, or Home Health settings.

Qualifications

The Physical Therapist would evaluate and treat patients, in accordance with the orders from the physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination, body alignment, to relieve pain, improve movement and function. The Physical Therapist will work as a member of a team providing support and information within the area of Physical Therapy. The Physical Therapist will provide therapy services in locations including, but not limited to, Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities or Home Health Care.

• Bachelor of Science or advanced degree in Physical Therapy
• Current state license/certification
• Current registration with the American Physical Therapy Certification Board
• Prefer minimum one (1) year experience in rehabilitation
• Excellent oral, written communication and interpersonal skills
• Willingness to provide therapy services in locations including, but not limited to, Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities or Home Health Care
